The BBC committed a "serious breach" of media regulations by failing to disclose that the voice of a documentary about the Gaza conflict was the offspring of a Hamas representative, as ruled by UK media regulator Ofcom.
An probe by the media regulator into the programme Gaza: How to Survive a Warzone has determined that the film was "materially misleading".
The head of the BBC had previously apologised, saying there had been "a serious error in relation to accuracy".
The authority has ordered the corporation to broadcast a evening announcement about its conclusions.
"Given this constitutes a significant violation of our regulations, we are instructing the corporation to broadcast a message of our determinations against it on BBC Two at 9pm, with a timing to be finalised," the statement read.
'Potential to Erode Confidence'
The watchdog's report stated: "Our probe determined that the film's failure to disclose that the narrator's male guardian held a role in the Hamas-administered authority was significantly deceptive.
"This implied that the viewers did not have essential details which may have been extremely pertinent to their assessment of the voice and the information he shared.
"Confidence is at the core of the relationship between a television network and its public, notably for a national broadcasting corporation such as the BBC.
"This oversight had the potential to damage the significantly high degrees of confidence that audiences would have put in a BBC factual programme about the Gaza conflict."
The film was withdrawn from iPlayer in February after the presenter's personal associations were revealed, and in July an broadcaster's internal assessment determined it violated the broadcaster's journalistic standards on accuracy.
A representative for the broadcaster stated: "The Ofcom ruling is in accordance with the findings of the corporation's inquiry, that there was a major oversight in the film in relation to the BBC's Editorial Guidelines on factual correctness, which corresponds to clause 2.2 of Ofcom's Broadcasting Code.
"We expressed regret for this and we agree with the authority's determination in complete. We will follow the penalty as soon as the schedule and language are determined."
